Weekend Physical Therapist – Home Health Visits

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist to perform weekend home health visits in the Tucson area, serving adult and geriatric clients across Pima and Pinal Counties.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, including range of motion, strength, balance, and safety assessments.
• Develop and implement individualized care plans with manual therapeutic exercises, gait training, and balance interventions.
• Continuously assess and revise care plans and collaborate with nursing staff on multidisciplinary plans.
• Educate clients and families on rehabilitative care to promote health, safety, and independent living.
• Accurately document all observations, interventions, and evaluations using a tablet‑based EMR system.

Required profile

• Current Arizona Physical Therapist license.
• Minimum of one year recent clinical experience as a licensed Physical Therapist.
• Graduation from an APTA‑approved PT program.
• Ability to work independently, manage time effectively, and possess strong interpersonal and problem‑solving skills.

Required skills

• AZ Physical Therapist licensure.
• Experience with electronic medical record (EMR) systems.
