Social Worker - PRN

About the role

The Social Worker provides bio‑psychosocial assessments, patient education, and crisis interventions while coordinating discharge plans and linking patients to community resources. This PRN position works within Ochsner’s multidisciplinary team to address emotional, social, cultural, and spiritual needs of patients and families.

Key responsibilities

• Conduct patient assessment interviews and develop individualized discharge plans.
• Identify and refer patients and families to appropriate community resources.
• Maintain accurate patient records and communicate critical information to the medical team.
• Participate in on‑call rotation and adapt communication style to diverse patient populations.

Required profile

• Master’s degree in Social Work (MSW) from a CSWE‑accredited program.
• Certified Social Worker (CSW) or Registered Social Worker (RSW); LMSW preferred or obtainable within 6 months.
• No prior experience required; two years in a medical setting preferred.
• Ability to work flexible schedules, including weekends and on‑call availability.

Required skills

• Proficiency with Windows‑style applications and data‑entry software.
• Effective verbal and written communication for multidisciplinary collaboration.
• Working knowledge of medical terminology and common disease entities.
