RN Transitional Care Manager (Adult and Pediatric Skilled)

About the role

Join BAYADA Home Health Care as a Transitional Care Manager, supporting adult and pediatric skilled clients across Massachusetts, New Hampshire, Rhode Island, and New York. You will act as a key liaison between referral sources, healthcare providers, and clients transitioning from acute and post‑acute settings to home care.

Key responsibilities

• Develop and maintain relationships with hospitals, skilled nursing facilities, physicians, case managers, and discharge planners.
• Educate referral partners, clients, and families on BAYADA’s home health services and benefits of home‑based care.
• Facilitate safe transitions for medically complex clients and provide post‑transition follow‑up to improve outcomes.
• Review medical documentation, assess eligibility, and coordinate admissions with service offices.
• Create and execute strategic referral development plans and track performance metrics.

Required profile

• Current, unrestricted RN license.
• Minimum five years of nursing, discharge planning, critical care, long‑term care, or related experience.
• Strong communication, presentation, and interpersonal skills.
• Ability to work independently, manage a large geographic territory, and prioritize multiple responsibilities.

Required skills

• Proficiency with Microsoft Office applications.
