RN Clinical Services Liaison – PRN Weekend

About the role

The RN Clinical Services Liaison provides clinical coordination of care from referral sources to the home setting, ensuring patients receive appropriate home infusion or health services. This PRN weekend position supports discharge planning and patient education.

Key responsibilities

• Develop and maintain relationships with referral sources such as hospital case managers, home health agencies, and physician offices.
• Identify appropriate candidates for home infusion or health care and assess caregiver ability.
• Coordinate in‑services, continuing education, and discharge planning activities for new patient referrals.
• Provide patient education and training on therapy, supplies, and medication connections.
• Attend office, sales, training meetings, and relevant trade shows as directed.

Required profile

• Registered Nurse diploma (Bachelor’s preferred) with a current RN license in the state of practice.
• Minimum 3 years of nursing experience.
• Basic Life Support (BLS) certification from the American Heart Association.
• Ability to work in a fast‑paced environment with frequent interruptions.

Required skills

• Computer proficiency for data entry and retrieval, including Windows‑style applications.
• Effective verbal and written communication.
• Knowledge of core therapy referrals and qualifying criteria (TPN, Enteral Nutrition, Inotropic Agents for Medicare).
