About the role

Join Cleveland Clinic Tradition Hospital’s team of caregivers to provide top‑quality patient care in Port St. Lucie. The RN Case Manager collaborates with inter‑professional teams to ensure effective transitional care coordination and patient advocacy.

Key responsibilities

Ensure compliance with accrediting bodies, state regulations, and CMS Conditions of Participation.
Promote interprofessional synchronization throughout the acute episode and complete effective handoffs to the next level of care.
Assess, identify, and mitigate barriers to patients’ transitions of care, developing individualized transitional care plans.
Participate in patient rounding activities, huddles, and committee meetings.

Required profile

Graduate of an accredited nursing program (Diploma, ADN, or BSN).
Current state licensure as a Registered Nurse (RN).
Basic Life Support (BLS) certification.
Three years of recent RN experience.

Required skills

Basic Life Support (BLS) certification.
Active RN licensure.
