RN Care Manager – Full-Time (0.9 FTE)

About the role

The RN Care Manager at Phelps Health coordinates care and services for assigned patient populations, ensuring effective utilization of health resources and appropriate placement across the care continuum. This full‑time position involves leadership within the Multidisciplinary Care Team and collaboration with various healthcare professionals.

Key responsibilities

• Promote effective utilization and monitoring of health care resources and ensure appropriate placement based on medical necessity.
• Lead the Multidisciplinary Care Team to achieve optimal clinical, financial, and resource outcomes.
• Develop goal‑directed care plans based on diagnosis, patient problems, co‑morbidities, and expected outcomes.
• Coordinate and communicate discharge plans with patients, families, and providers, considering social determinants of health.
• Follow regulatory guidelines, inform patients of their rights, and manage denial and appeal processes.

Required profile

• Graduate of an accredited School of Nursing or Social Work, or equivalent case management experience with a bachelor’s degree.
• At least 2 years of applicable clinical experience.
• Missouri or Compact RN license (if applicable) and BLS certification.
• Case Management certification preferred.

Required skills

• Utilization management and discharge planning experience.
• Knowledge of third‑party payors and regulatory guidelines.
• Ability to lift up to 35 lb and perform frequent walking and standing.
