RN Care Coordinator - Neurology

About the role

The RN Care Coordinator will work within the Fairview Neuro department at Cleveland Clinic Fairview Hospital, providing outreach and coordination for high‑risk neurology patients on a part‑time schedule.

Key responsibilities

• Identify patients needing ongoing care coordination and define the scope of involvement.
• Conduct targeted outreach to high‑risk patients, using registries and patient lists to manage populations.
• Perform comprehensive clinical assessments covering disease‑specific, medical, behavioral, pharmacy, social and end‑of‑life needs.
• Collaborate with interdisciplinary teams to develop goals, plans, and interventions that maximize patient outcomes.
• Manage transitions of care, close care gaps, and serve as the primary patient contact and liaison.

Required profile

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification.
• Three to five years of nursing experience.

Required skills

• Basic Life Support (BLS) certification.
• Proficiency with technological tools such as registries, patient lists, and care team tabs for population management.
