RN Care Coordinator – Medical Oncology

About the role

The RN Care Coordinator provides comprehensive coordination for high‑risk oncology patients at Fairview Hospital, ensuring timely care across the continuum and supporting optimal outcomes.

Key responsibilities

• Identify patients with ongoing coordination needs and define the scope of involvement.
• Conduct targeted outreach to high‑risk patients to ensure efficient care delivery.
• Utilize registries and patient lists to perform comprehensive clinical assessments.
• Collaborate with interdisciplinary teams to develop goals, plans, and interventions.
• Perform reassessments, close care gaps, and serve as primary patient contact.
• Manage transitions of care, identify barriers, and partner with social work, pharmacy, palliative care, and other teams.

Required profile

• Graduate from an accredited nursing program with current RN licensure.
• Three to five years of nursing experience, preferably in oncology or related specialty.
• Basic Life Support (BLS) certification.
• Ability to work the 8:00 am‑4:30 pm shift, Monday‑Friday.

Required skills

• Clinical assessment of disease‑specific, age‑specific, medical, behavioral, pharmacy, social, and end‑of‑life needs.
• Use of electronic tools such as registries, patient lists, and care team tabs for population management.
