Registered Nurse Admissions – Weekend Days Home Care with $15k Sign‑On Bonus

About the role

The Lehigh Valley Health Network seeks a Registered Nurse (RN) Admissions Nurse to provide weekend day coverage for home‑care patients. The role involves determining appropriate levels of care, completing CMS data collection, and creating individualized care plans in collaboration with physicians and the interdisciplinary team.

Key responsibilities

• Perform comprehensive initial and follow‑up assessments covering physical, psychological and environmental factors.
• Develop and document individualized plans of care based on physician orders and current scientific findings.
• Communicate significant changes in patient status promptly to physicians and the care team.
• Provide patient and caregiver education on end‑of‑life issues, palliative care, chronic disease management, pain control, and discharge planning.

Required profile

• Graduated from an NLN‑approved nursing program with a current RN license in Pennsylvania.
• At least one year of recent direct‑care experience in a healthcare setting.
• Ability to travel to patient assignments and work independently as part of a team.
• Valid driver’s license for Pennsylvania (or New Jersey) and ability to lift 40‑50 lb.

Required skills

• Basic Life Support (BLS) certification.
• Proficiency with multiple computer applications and electronic medical record (EMR) platforms.
