About the role

Commonwise is seeking a PRN Registered Nurse to join its care management and home care team, providing non‑clinical support to older adults and coordinating services.

Key responsibilities

Provide senior housing and relocation assistance, supportive consulting, and family relationship management.
Manage healthcare crises, coordinate healthcare services, and conduct home safety evaluations.
Serve as account manager for clients receiving home care services and supervise nurse‑aides delivering direct care.

Required profile

Registered Nurse licensed in Virginia.
At least 5 years of nursing experience.
Excellent written and verbal communication skills.
Interest in a non‑clinical work environment and strong problem‑solving abilities.
