Home Health LPN – Tampa or Sarasota, FL

About the role

BayCare HomeCare is seeking a Licensed Practical Nurse (LPN) to provide hands‑on, in‑home care for patients in the Tampa Bay or Sarasota/Charlotte County areas. The role involves delivering high‑quality nursing services, following clinical protocols, and collaborating closely with a Registered Nurse to ensure optimal patient outcomes.

Key responsibilities

• Deliver patient care in the home setting, including IV infusions, infusion pump management, and tube feedings under RN supervision.
• Develop and implement teaching plans for patients and caregivers as directed by the RN.
• Follow and adjust the plan of care based on patient response and status changes.
• Administer medications, evaluate effectiveness, and document care according to nursing standards.

Required profile

• Active Florida LPN license and valid Florida driver’s license (required within 30 days of hire).
• Associate’s degree or nursing diploma.
• Minimum 1 year of LPN experience, preferably in home care.
• Basic Life Support (BLS) certification.

Required skills

• IV infusion administration.
• Operation of infusion pumps.
• Tube feeding management.
• Medication administration and evaluation.
• Basic Life Support (BLS) techniques.
