Physical Therapist - Home Health Visits

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist for per‑diem home health visits serving adult and geriatric clients in Monroe County. You will join a multidisciplinary team to provide rehabilitative care that enables clients to remain safely at home.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, including range of motion, strength, balance, and safety assessments.
• Implement wound‑prevention and wound‑care interventions using evidence‑based dressings.
• Develop and adjust individualized care plans with manual therapeutic exercises, gait training, and balance work.
• Educate clients and families on rehabilitative care to promote independence.
• Document all observations, interventions, and evaluations using a tablet‑based electronic medical record system.

Required profile

• Current Pennsylvania Physical Therapist license.
• Minimum one year of recent clinical experience as a licensed PT.
• Graduation from an APTA‑approved PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving abilities.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills.
