Patient Flow Center Coordinator

About the role

The Patient Flow Center Coordinator operates the bed board for patient placement, assists with psychiatric placement, dispatches transportation, and supports tele‑medicine calls. This full‑time, day‑shift position works within a collaborative, patient‑focused environment.

Key responsibilities

• Coordinate transportation for all transport orders acting as a dispatcher.
• Respond to system‑wide acute behavioral health placement requests and submit electronic referrals.
• Provide customer service to physicians, nurses, administrators and colleagues via phone and in‑person.
• Facilitate time‑sensitive tele‑medicine consultations and create tickets in Epic.
• Assign beds across campuses, managing the bed board for community hospitals.

Required profile

• Completion of an accredited LPN program or a bachelor’s degree.
• Minimum 2 years of LPN experience (or bachelor’s degree in lieu of experience).
• Current LPN license in the state of practice.
• Ability to work extended shifts of 8, 10, or 12 hours.

Required skills

• Proficiency with computers, software and web‑based applications.
• Effective verbal and written communication.
• Excellent telephone customer service and multitasking abilities.
• Self‑directed, motivated with minimal supervision.
