Patient Care Assistant – Part-Time (Transitional Care)

About the role

Phelps Health is seeking a Part‑Time Patient Care Assistant to work in its Transitional Care Facility. Under the supervision of a licensed nurse, you will provide direct patient care and support a safe, high‑quality environment for patients.

Key responsibilities

• Follow hospital policies on infection prevention, safe patient handling, and notify nurses of any changes in patient condition.
• Perform basic patient care tasks such as vital signs, glucometer checks, toileting, feeding, bathing, and activity assistance.
• Collaborate with nursing staff to respond quickly to patient requests and enhance the patient experience.
• Document all patient care activities accurately to support clinical decision‑making.
• Communicate pertinent patient information to the care team to improve outcomes.

Required profile

• High school diploma or GED (preferred).
• One year of related experience (preferred).
• Certified Nursing Assistant (CNA) credential in Missouri (preferred) or willingness to attend PCA Academy training.
• Ability to lift up to 35 lb and perform frequent walking, standing, and patient handling tasks.

Required skills

• Certified Nursing Assistant (CNA) – Missouri (preferred).
• Basic Life Support (BLS) certification obtained within 90 days of hire (training provided).
