Medical Social Worker – Home Health

About the role

BAYADA Home Health Care is seeking a Per Diem Medical Social Worker to provide home health visits for patients in Berkeley and Dorchester Counties. You will join a multidisciplinary team delivering rehabilitative and skilled nursing care, helping clients remain safely at home.

Key responsibilities

• Conduct home visits to clients within the assigned geographic territory.
• Perform clinical social evaluations, develop intervention plans, and document findings using a tablet.
• Identify and refer clients and families to appropriate community resources and act as an advocate navigating the system.
• Collaborate with the multidisciplinary BAYADA team to develop total plans of care.
• Maintain contracts with public and private agencies and educate patients and families on resources for independent living.

Required profile

• Licensed by the South Carolina Board of Social Work Examiners at LMSW or LISW level.
• Master’s degree in Social Work with at least 2 years of community‑based experience.
• Strong interpersonal and communication skills, ability to work independently and manage time effectively.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills, including use of tablet devices for documentation.
