Manager – Care Management

About the role

The Manager – Care Management leads the case/care management team in Monroe, ensuring evidence‑based resources improve patient quality and outcomes. This position collaborates with case managers, physicians, and ancillary staff to coordinate care, discharge planning, and transition services.

Key responsibilities

• Manage daily department operations, staffing levels, and assignments.
• Develop and maintain relationships with primary care physicians, clinical teams, and physician advisors to promote coordinated, evidence‑based care.
• Research, select, and adapt best‑practice findings to enhance patient outcomes and remove barriers.
• Analyze payer/partner data, meet plan objectives, and drive process improvements.
• Interview, train, motivate, evaluate, and monitor staff performance.

Required profile

• Registered Nurse diploma or Master’s Degree in Social Work.
• At least 3 years of experience in case management, disease management, or utilization management.
• Current Louisiana RN license or Licensed Master Social Worker (LMSW); CCM certification preferred.
• Strong leadership, delegation, organizational, and time‑management abilities.
