About the role

The Cleveland Clinic Health System is seeking a Licensed Practical Nurse (LPN) to join the Primary Care team at the Green Health and Wellness Center in Uniontown. You will work under the direction of Registered Nurses and physicians to deliver patient‑first care in an ambulatory setting.

Key responsibilities

Provide patient care activities under RN and physician direction.
Collect and document objective and subjective patient data.
Complete nursing assessments and report findings.
Implement nursing care plans, administer medications or treatments, and monitor patient responses.
Observe patients for adverse reactions and perform routine laboratory tests.
Educate patients and family members about health conditions and care plans.

Required profile

Graduate of an approved practical nursing program.
Current Ohio Licensed Practical Nurse (LPN) license.
Basic Life Support (BLS) certification through AHA or Red Cross.
One year of patient care experience preferred.

Required skills

Ohio LPN licensure.
Basic Life Support (BLS) certification.
