Hospice RN (On-Call) – Night Shift

About the role

James River Hospice is seeking a compassionate Hospice RN for on‑call night shifts in Lynchburg, VA. You will manage hospice case care for assigned patients in their homes, coordinating with an interdisciplinary team to ensure optimal patient experience.

Key responsibilities

• Conduct initial assessments of patients and families to determine hospice needs.
• Develop and modify care plans based on nursing diagnoses and palliative actions.
• Coordinate care with the clinical team, supervising LPNs and hospice aides.
• Provide pain management support and spend quality time with patients.
• Monitor, record, and report symptoms and changes in condition.
• Obtain physician orders as required and participate in on‑call duties.

Required profile

• Current Registered Nurse license in Virginia.
• At least one year of hospice experience.
• Graduate of an accredited nursing school.
• Valid driver’s license and reliable vehicle for work use.
