Bilingual Intake and Referral Coordinator

About the role

You will be the first voice patients hear when they reach out for home health care, guiding referrals from initial contact to scheduled nurse visits. The position blends call handling, documentation verification, and scheduling in a fast‑moving, remote environment.

Key responsibilities

• Serve as the primary point of contact for incoming patient referrals and inquiries.
• Verify patient eligibility, secure authorizations, and ensure all referral documentation is complete.
• Coordinate intake appointments by matching patient needs with nurse and field staff availability.
• Maintain organized records, follow established workflows, and promptly escalate any issues.
• Communicate clearly and warmly with Spanish‑speaking patients.

Required profile

• Fluent in English and Spanish, both spoken and written.
• Excellent organizational and triage skills to prioritize multiple referrals.
• Strong collaborative attitude and ability to work independently while staying connected to the team.
• Willingness to work US time‑zone hours and have a reliable technical setup.
