Home Care Occupational Therapist

About the role

The Home Care Occupational Therapist provides evaluation and treatment for patients with physical, emotional, cognitive, and psychosocial deficits, enabling them to safely and independently perform daily activities. This PRN position works within an interdisciplinary home‑care team at Union Hospital, supporting patients and caregivers alike.

Key responsibilities

• Perform initial and ongoing assessments of patients’ conditions.
• Develop, revise, and evaluate individualized plans of care in collaboration with patients, families, and the interdisciplinary team.
• Deliver therapy interventions using standard occupational therapy techniques appropriate to the patient’s age and condition.
• Supervise occupational therapy assistants, students, and other support personnel.
• Maintain complete, up‑to‑date documentation of patient status, interventions, and outcomes.
• Participate in interdisciplinary assessments and, when needed, hospice team meetings.

Required profile

• Bachelor’s degree from an AOTA‑accredited Occupational Therapy program.
• Current state licensure as an Occupational Therapist.
• Current Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and automobile insurance meeting policy requirements.
• Ability to meet physical demands of the role, including walking, lifting, and manual dexterity.
