Habilitation Manager, IDD

About the role

BAYADA Home Health Care is seeking a qualified Habilitation Manager to join its Charlotte office. In this position you will act as an internal case manager and advocate for clients in the habilitation program, working closely with clinical and care teams.

Key responsibilities

• Collaborate with the Clinical Manager to manage client cases and participate in Person‑Centered Planning processes.
• Conduct regular client visits to evaluate progress against Individual Support Plans and ensure services are delivered as defined.
• Supervise Habilitation Technicians in accordance with policy and state regulations.
• Maintain communication with Case Managers and Care Coordinators and support client‑specific orientations.

Required profile

• Bachelor’s degree required.
• Minimum of 2 years post‑degree experience working with the I/DD population.
• Qualified Professional status.
• Prior supervisory experience preferred.
• Strong communication and interpersonal skills.

Required skills

• Experience with Person‑Centered Planning and Individual Support Plans.
• Ability to evaluate client progress and adjust care plans.
