Director of Coordination

About the role

The Director of Coordination leads all care coordination operations for a high‑volume, New York‑licensed home care agency. This senior operational role ensures uninterrupted patient coverage, efficient caregiver utilization, and compliance with NYS regulations while driving performance across coordination teams.

Key responsibilities

• Lead Care Coordinators and Coordination Supervisors, enforcing performance, responsiveness, and documentation standards.
• Maintain full staffing of authorized cases, manage call‑outs, emergencies, overtime limits, and difficult‑to‑service cases.
• Oversee live‑in and split cases, schedule adjustments, and ensure compliance with labor laws and agency policy.
• Partner with HR, Payroll, Recruitment, Nursing, Intake, Authorizations, and Retention to resolve escalations and staffing challenges.
• Track and report key coordination metrics such as unstaffed hours, overtime, response times, and complaints, and develop improvement plans.

Required profile

• Minimum 2 years of experience in home care coordination.
• 5 + years in a leadership or management role within Home Care.
• Strong knowledge of NYS home care regulations and labor considerations.
• Proven ability to manage high‑volume, fast‑paced coordination teams.

Required skills

• Experience with HHA Exchange.
