Diabetes Care Specialist (RN/Registered Dietitian) – Full Time

About the role

This full‑time position supports Ochsner’s nationally recognized Diabetes Self‑Management Education and Support (DSMES) program. The specialist works directly with diabetic patients, assessing health status, medication use, and psychosocial factors to develop individualized education and care plans.

Key responsibilities

• Assess patients’ physical health, medications, and psychosocial issues to identify education and clinical intervention needs.
• Develop and implement personalized diabetes care plans in collaboration with medical providers and the care team.
• Provide comprehensive education on disease process, complications, nutrition, medication, exercise, foot care, and glucose monitoring.
• Serve as a resource on diabetes care for healthcare professionals, staff, clinicians, and the community.
• Recommend pharmacological options, medical devices (e.g., insulin pumps, continuous glucose monitors) and alternative treatments based on patient needs.

Required profile

• Registered Nurse diploma or bachelor’s degree in a related field (master’s preferred).
• Minimum 3 years of direct patient care experience.
• Certification as a Diabetes Care and Education Specialist (CDCES) preferred.
• Ability to work evenings and weekends and maintain reliable transportation.
