Community Health Navigator

About the role

Stony Brook Clinical Practice Management (CPMP) is hiring a full-time Community Health Navigator in Commack, NY, working Monday to Friday. The role supports administrative operations and patient-facing interactions, acting as a liaison between Care Managers and patients and families, linking patients with community resources, medical appointments, education and transportation. It works under direct supervision as part of an interdisciplinary Care Management team.

Key responsibilities

• Support daily operations such as team schedules and new patient referrals, and help patients adhere to their care plan through medical visits.
• Use motivational interviewing to help patients overcome barriers and obtain resources for their social, medical and personal needs.
• Maintain knowledge of community resources and agencies for the assigned locality and update them as needed.
• Follow productivity metrics, quality standards and documented care plans, and close cases in a timely way.
• Follow up with patients by home visits or phone calls as assigned, and track utilization and care data to identify trends and improve outcomes.
• Do field work as needed, help with special projects and build strong relationships with internal and external customers.

Required profile

• Bachelor's degree in Health Services or a Community Health Worker certification.
• Own reliable transportation and the ability to drive safely.
• Excellent interpersonal, customer service, communication (written and verbal) and teaching skills.
• Preferred: degree in business administration, healthcare management or a related field; one year of supervisory experience; experience in academic medicine or a large healthcare organization; pursuing a degree in finance, accounting or business analytics.

Required skills

• Proficiency in word processing, spreadsheets, graphics and database programs, including Microsoft Word, Excel, PowerPoint and Outlook.
