Hospice Registered Nurse (RN) - PRN

About the role

Amedisys, part of the Optum family of businesses, offers home health, hospice and community-based care. As a per diem Hospice Registered Nurse, you will provide compassionate, patient-centered care by assessing needs, coordinating individualized care plans, managing symptoms, educating families and collaborating with the clinical team. The schedule is flexible and the territory covers Crawford and Venango Counties; hospice experience is preferred.

Key responsibilities

• Perform initial nursing assessments on new admissions
• Complete, coordinate and revise the total plan of care, maintaining continuity of care with the care team
• Communicate patient progress regularly to the clinical manager, physician and care team
• Maintain accurate, timely patient care progress records in line with care center policies
• Record pain and symptom management changes and outcomes
• Educate families on the patient's condition, needs and care
• Make referrals to other hospice disciplines or document the rationale for not doing so
• Perform on-call duties and other duties as assigned

Required profile

• Current RN license for the state(s) of assignment
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance
• Preferred: 1+ year of nursing experience and hospice experience
