Clinical Pharmacist - Inpatient PRN

About the role

The Clinical Pharmacist – Inpatient PRN supports the inpatient pharmacy team at Medina Hospital, providing comprehensive clinical and operational pharmacy services with a focus on orthopedic care. The role works collaboratively with pharmacists, physicians, nurses, and other interdisciplinary team members to ensure safe, effective, and evidence‑based medication therapy.

Key responsibilities

• Verify and process medication orders and dispense medications in compliance with state and federal regulations.
• Perform clinical chart reviews, medication reconciliation, and enter/clarify orders in the electronic health record.
• Respond to medication‑related questions from caregivers throughout the hospital.
• Evaluate medication appropriateness, resolve formulary and therapeutic issues, and partner with the care team to optimize patient outcomes.
• Educate and train students, residents, other caregivers, and patients.

Required profile

• PharmD or Bachelor’s of Science in Pharmacy from an accredited college of Pharmacy.
• Current state licensure as a Registered Pharmacist (RPh) or eligibility for reciprocation within 90 days of hire.
• Current Basic Life Support (BLS) certification and completion of an immunization administration course or certification within 90 days of hire.
• Ability to work variable PRN shifts between 6:30 am and 9:30 pm, including at least every fourth weekend.
