Clinical Pharmacist - Inpatient - 7 on / 7 off, Evenings

About the role

The Clinical Pharmacist – Inpatient provides comprehensive pharmacy services for hospitalized patients at Cleveland Clinic Fairview Hospital. The position works a 7‑on/7‑off evening schedule (1:00 pm‑11:30 pm) and supports both clinical and operational pharmacy functions.

Key responsibilities

• Verify and clarify medication orders, ensuring accuracy and regulatory compliance.
• Evaluate drug therapy, optimize dosing, and monitor for interactions and therapeutic appropriateness.
• Provide pharmacokinetic consultations and support sterile product preparation.
• Participate in interdisciplinary rounds when scheduled in a clinical capacity.
• Provide operational pharmacy coverage as needed, maintaining medication safety.

Required profile

• PharmD or Bachelor of Science in Pharmacy from an accredited college.
• Current state licensure as a Registered Pharmacist (or eligible for reciprocity within 90 days).
• Basic Life Support (BLS) certification and immunization administration course completion.
• Advanced Cardiovascular Life Support (ACLS) and/or Pediatric Advanced Life Support (PALS) certification.
• Physical ability to stand, walk, lift up to 30 lb, and perform manual dexterity tasks.
