Clinical Navigator – M-Power Program (PRN)

About the role

The Clinical Navigator for the M‑Power Program provides trauma‑informed, one‑on‑one support to patients preparing for labor, delivery and the postpartum period. This PRN position is designed for current or former Labor and Delivery nurses with specialized training in trauma‑informed care, working at Akron General Medical Center.

Key responsibilities

• Facilitate communication among nursing staff, physicians and the multidisciplinary team.
• Assist patients in accessing necessary treatments, services and post‑discharge support.
• Review surgical, diagnostic and interventional procedures and explain expected outcomes to patients.
• Prepare families for realistic initial visits after procedures and review appropriate rehabilitation programs.
• Conduct daily rounds on designated patients, assess status and address concerns.
• Assist patients and families with financial and administrative issues by referring to appropriate resources.
• Follow up on post‑discharge phone calls and coordinate care with support services such as Global Patient Services, Medical Concierge and Pastoral Care.
• Facilitate patient transfers as needed.

Required profile

• Graduate from an approved School of Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification.
• Minimum three years of nursing experience with emphasis on a specialty.
• Bachelor of Science in Nursing (BSN) preferred.
