Clinic Based Community Health Worker

About the role

Share is seeking a Clinic Based Community Health Worker to work on‑site at the PeaceHealth Community Hub in Clark County. The CHW will connect patients experiencing homelessness with health and social services, using a whole‑person approach to reduce barriers and improve overall well‑being.

Key responsibilities

• Receive and triage referrals in real time, coordinating urgent care with hospital staff.
• Engage patients and community partners, establishing strong connections to resources such as food assistance, housing support, and behavioral health services.
• Assist individuals in navigating complex systems, scheduling appointments, tracking progress, and providing coaching toward self‑identified goals.
• Maintain accurate records of patient interactions, services provided, and outcomes in the Care Coordination System, ensuring compliance with HIPAA.
• Collaborate with internal teams, community‑based organizations, and public health agencies to deliver coordinated services.

Required profile

• Bachelor’s degree in public health, social work, healthcare, or related field with at least three years of relevant experience, or five years of experience without a degree.
• Certification as a Community Health Worker or ability to obtain certification within the required timeframe.
• Valid driver’s license, reliable transportation, and auto insurance.
• Ability to pass background checks, drug screening, health screenings, and hospital orientation.

Required skills

• Proficiency with Microsoft Office or Google Workspace.
• Experience using electronic health record (EHR) systems for documentation.
• Strong organizational and documentation skills, including attention to detail and data entry.
