Case Manager – Outpatient Substance Abuse & Mental Health Clinic

About the role

The Case Manager provides direct support to individuals receiving services at Clinton Counseling Center, a private non‑profit outpatient substance abuse and mental health clinic in Mt. Clemens, MI. This contractual position offers flexible hours, local community travel, and focuses on connecting clients with resources, advocacy, and coordinated care.

Key responsibilities

• Assessment and reassessment of clients assigned to a caseload.
• Planning and facilitating person‑centered service plans.
• Developing and updating an Individual Plan of Service (IPOS) at least annually.
• Linking clients to mental health, community services, and supports.
• Brokering with service providers and assisting with entitlements or legal representation.
• Coordinating with other care providers and documenting clinical information promptly.
• Participating in case reviews and monitoring services provided.
• Providing crisis intervention and discharge planning.
• Outreach and networking with other service providers as needed.

Required profile

• Bachelor’s degree in Social Work, Counseling or Psychology; Master’s preferred.
• Relevant certifications such as CADC/CAADC or MCBAP Development Plan.
• Valid driver’s license and personal vehicle for community travel.
• Ability to pass a criminal background check.
• Supervision provided by a fully licensed LPC or LMSW.
