Care Coordination Case Manager

About the role

Sailor Health is building a virtual care platform for older adults, combining behavioral health, advocacy, wellness classes, and AI tools. We are seeking a full‑time, remote Care Coordination Case Manager to ensure continuity of care for high‑acuity patients who need services beyond virtual outpatient therapy.

Key responsibilities

• Identify patients who are too high‑acuity or out of scope for the virtual model by partnering with therapists, clinical leads, and the care navigation team.
• Assess patients and caregivers via phone or video to understand needs, preferences, insurance coverage, and barriers to care.
• Plan individualized transition‑of‑care pathways, matching patients to appropriate services such as psychiatry, IOP/PHP, inpatient treatment, geriatric psychiatry, memory care, substance‑use treatment, primary care, or community services.
• Execute the plan by placing referrals, scheduling appointments, transmitting records with authorization, and confirming patient attendance.

Required profile

• Bachelor’s degree in social work, nursing, psychology, public health, or related field (master’s a plus).
• 2+ years of experience in case management, care coordination, discharge planning, utilization management, or referral coordination within behavioral health or healthcare.
• Experience with older adults, Medicare, or Medicare Advantage strongly preferred.
• Clinical licensure or certification (LMSW, LSW, LCSW, RN, LPC, CCM, ACM) is a plus but not required.
• Must reside in the United States and be authorized to work there.

Required skills

• Working knowledge of behavioral health levels of care (outpatient, IOP, PHP, inpatient, residential).
• Comfort with telehealth platforms and electronic medical records (EMRs).
• Exceptional written and verbal communication and relationship‑building abilities.
