Behavioral Health Integration Clinician – Associate Level

About the role

Join Mindoula Health’s Collaborative Care Program as an Associate‑level Behavioral Health Integration Clinician. You will work within a team‑based model to embed behavioral health services into primary care, providing short‑term, evidence‑based interventions to adults with chronic medical and behavioral health needs.

Key responsibilities

• Screen and assess patients for mental health and substance use disorders.
• Deliver brief, evidence‑based interventions (e.g., behavioral activation, problem‑solving therapy, motivational interviewing).
• Support medication adherence, monitor side effects, and provide education and self‑management support.
• Track clinical response via in‑person, video, or phone visits and adjust care plans as needed.
• Document care in the EHR according to model workflows and participate in weekly psychiatric case consultations.

Required profile

• Master’s degree in Social Work, Counseling, Psychology, or a related field.
• Associate‑level license (e.g., Clinical Social Work Associate) and at least 3 years of experience with adult populations facing chronic medical and behavioral health challenges.
• Located in Oregon; majority of work is remote with hybrid presence at the Corvallis clinic.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Experience using telehealth platforms and managing multiple technology applications.
