Behavioral Health Therapist

About the role

The Behavioral Health Therapist joins Mindoula Health’s Collaborative Care Program in Oregon, providing integrated behavioral health services within primary care. Working on a hybrid schedule, the clinician supports patients with mental health and substance‑use disorders while coordinating with medical providers and psychiatric consultants.

Key responsibilities

• Screen, assess, and develop treatment plans for patients with mental health and substance‑use disorders.
• Deliver brief evidence‑based interventions (e.g., behavioral activation, motivational interviewing) via in‑person, video, or telephone visits.
• Support psychotropic medication management, monitor adherence, side effects, and treatment effectiveness.
• Track patient progress, document encounters in the EHR, and coordinate referrals to additional psychosocial services.
• Participate in weekly case consultations with the psychiatric consultant and communicate recommendations to the medical provider.

Required profile

• Master’s degree in Social Work, Psychology, Counseling, Rehabilitation, or a related field.
• 3‑5+ years of experience in behavioral health intervention, case management, or community care coordination.
• Active independent clinical license in Oregon (LCSW, LPC, LMFT, or Licensed Psychologist).
• Experience working with diverse populations, including older adult Medicare patients with chronic medical and behavioral health challenges.

Required skills

• Proficiency with electronic health records (EHR) systems.
• Experience using scheduling, remote phone, and video conferencing platforms.
