Remote US Licensed Practical Nurse (LPN) – Outbound Care Management

About the role

We are looking for a Licensed Practical Nurse (LPN) to join our remote Chronic Care Management team. The role involves outbound, call‑center‑style patient outreach to support ongoing care plans and ensure continuity of care.

Key responsibilities

• Make outbound calls to patients as part of chronic care management services.
• Conduct monthly outreach and follow‑up according to established care plans.
• Identify patient needs, barriers, and condition changes, escalating when necessary.
• Document interactions accurately and meet productivity and quality targets.
• Participate in virtual meetings using Google Workspace tools.

Required profile

• Active LPN license with a Compact State License (New York or California required as applicable).
• Experience in nursing, care management, patient outreach, or call‑center environments preferred.
• Strong verbal and written communication skills and ability to handle high‑volume outbound calls.
• Reliable high‑speed internet and a private, HIPAA‑compliant workspace.

Required skills

• Proficiency with Google Workspace (Gmail, Google Meet, Google Suite).
• Basic computer skills, including typing and navigating multiple applications.
• Ability to use company‑provided desktop computer and dual‑monitor setup.

What we offer

• Base pay of $18.25 per hour (+ $1.00 per hour bilingual differential where applicable).
• Performance‑based KPI incentive up to $400 per month.
