About the role

Lehigh Valley Health Network is seeking a full‑time Registered Nurse to provide home health care to patients in the Hazleton area. The role includes a $15,000 sign‑on bonus and offers the chance to work within a nationally recognized health system.

Key responsibilities

• Perform comprehensive subjective and objective assessments using standardized tools, evaluating physical, psychosocial, and environmental factors.
• Deliver patient care based on physician‑established medical plans, adjusting the plan of care as needed.
• Collaborate with interdisciplinary teams to address psychosocial needs of patients, families, and caregivers.
• Provide education on end‑of‑life issues, palliative care, chronic disease management, pain management, and discharge planning.
• Guide ancillary personnel and maintain professional nursing standards.

Required profile

• Specialized Diploma in Nursing with at least 1 year of recent direct‑care experience.
• Current Pennsylvania RN license (or obtain within 30 days of hire).
• Basic Life Support certification (American Heart Association) within 30 days of hire.
• Valid Pennsylvania driver’s license (or obtain upon hire).
• Ability to work independently, handle crisis situations, and communicate effectively.

Required skills

• Basic computer skills.
• Patient assessment and documentation.
• Knowledge of home‑care conditions of participation and patient education techniques.
