Registered Nurse Case Manager (PRN)

About the role

Limitlessli is seeking a Registered Nurse Case Manager to provide skilled nursing care to patients in Indiana on an intermittent, PRN basis. The role involves delivering comprehensive nursing services in patients' homes while coordinating with multidisciplinary teams.

Key responsibilities

• Conduct initial evaluation visits, regularly reassess patient needs, and develop or revise care plans.
• Document clinical notes, OASIS assessments, and discharge summaries in a timely manner.
• Coordinate services, process orders, and communicate changes to physicians and care teams.
• Supervise and mentor other nursing personnel and lead patient care conferences.
• Provide on‑call coverage nights, weekends, and holidays as assigned.

Required profile

• Graduate of an accredited nursing program; BSN preferred.
• Minimum two years of clinical nursing experience, preferably in home health or home care.
• Active RN licensure with ability to obtain additional state licenses.
• Experience caring for medically fragile clients and addressing medical, social, and behavioral needs.

Required skills

• Proficiency with OASIS assessments and documentation.
• Knowledge of ICD‑10 coding for accurate billing and reporting.
