Pharmacist – Long-Term Care (Tyler, TX)

About the role

PharMerica is seeking a dedicated Pharmacist to join its non‑retail pharmacy team in Tyler, TX. You will support long‑term care and senior living facilities by ensuring safe, effective medication therapy for residents.

Key responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and administration route.
• Maintain accurate resident medication records, including clinical and laboratory data.
• Order, evaluate, and discuss necessary laboratory tests for high‑risk conditions.
• Collaborate with the healthcare team to provide clinical and technical support.
• Document all communications with physicians, nurses, residents, and families in the medical record.
• Implement policies for safe procurement, storage, distribution, use, and disposal of drugs.
• Maintain required documentation in compliance with federal and state regulations.
• Report medication‑related errors to the Pharmacy Manager and assist staff with safe medication practices.
• Perform additional tasks as assigned.

Required profile

• Bachelor’s degree in Pharmacy (Pharm D preferred).
• At least one year of experience in long‑term care pharmacy.
• Active Texas RPh license.
• Detail‑oriented, goal‑driven team player with strong oral communication skills.
