About the role

The Lehigh Valley Health Network is seeking a dedicated Registered Nurse to join its hospice admissions team on weekends. This position focuses on assessing patients for hospice eligibility, creating individualized care plans, and providing end‑of‑life support while enjoying a $10,000 sign‑on bonus.

Key responsibilities

• Perform comprehensive initial and follow‑up assessments covering physical, psychological, and environmental factors.
• Complete CMS‑required hospice holistic assessments with high accuracy to ensure quality outcomes and appropriate reimbursement.
• Develop individualized plans of care based on physician orders and current scientific findings.
• Educate patients, families, and staff on palliative care, advance directives, pain management, and symptom control.
• Collaborate with physicians and the interdisciplinary team to adjust care as patient status changes.

Required profile

• Bachelor’s or Associate’s degree in Nursing with at least 1 year hospice experience (or 2 years acute care/hospice experience).
• Current RN license, BLS certification, and Pennsylvania driver’s license.
• Ability to travel within the service area and work independently.
• Strong communication skills for difficult end‑of‑life conversations.

Required skills

• Basic Life Support (BLS) – American Heart Association.
• Knowledge of Hospice Conditions of Participation and CMS regulations.
• Certified Hospice and Palliative Nurse (CHPN) preferred within 3 years.
