Travel Physical Therapist – Home Health (Palestine, TX)

About the role

Join a travel Physical Therapist assignment in Palestine, TX, delivering skilled home health services across East Texas. This contract position offers a change of scenery while supporting patients in their homes.

Key responsibilities

• Conduct home health physical therapy evaluations and start‑of‑care visits.
• Perform 30‑day reassessments, ongoing treatment visits, and discipline discharges.
• Complete agency discharges and document care per agency standards.
• Travel between patient homes in Crockett, Grapeland, Elkhart, and Palestine with mileage reimbursement.

Required profile

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an APTA‑approved program.
• Active Texas Physical Therapy license at start of assignment.
• Home health experience, including evaluations, start‑of‑care, reassessments, and treatment visits.
• Ability to work as an independent contractor.
• BLS certification from AHA or ARC may be required.

Required skills

• Familiarity with the HCHB electronic medical record system.
• BLS certification.
