Telehealth Nurse – Access Center (Hybrid) – Boynton Beach, FL

About the role

Join a dynamic Access Center team in Boynton Beach, Florida, providing remote patient care through telehealth. The role focuses on clinical decision‑making, patient education, and coordination with local providers to ensure safe, compassionate care.

Key responsibilities

• Review physician messages in the electronic medical record and triage urgent situations.
• Contact patients by phone for evaluation, referral, or emergency care as needed.
• Route non‑urgent messages according to established guidelines.
• Assist providers with patient outreach regarding test results and follow‑up instructions.
• Deliver telephonic education on chronic conditions such as diabetes, hypertension, asthma, COPD, and heart failure.
• Document all interactions accurately in the EMR system.

Required profile

• Bachelor of Science in Nursing.
• 3‑5 years of clinical experience, preferably in ambulatory or emergency settings with telephone triage.
• Unrestricted RN licenses in New York and Florida at time of hire.
• Strong critical‑thinking and independent decision‑making abilities.
• Excellent written and verbal communication skills.

Required skills

• Proficiency with Microsoft Office suite.
• Experience using Electronic Medical Record systems, preferably Epic.
