Speech Therapist (SLP) - Home Health, Williamsburg, VA

About the role

Commonwealth Home Care provides home health care to clients in their own homes, and is seeking a Speech-Language Pathologist (SLP) to deliver pathology services on an intermittent basis according to physician orders. The role is scheduled Monday to Friday and involves working with clients in their homes.

Key responsibilities

• Evaluate the client's level of functioning and recommend ways to improve hearing ability
• Provide initial and ongoing comprehensive assessments, including OASIS assessments
• Establish a speech therapy treatment plan with the physician and revise it as needed
• Help develop and implement the interdisciplinary client care plan for speech therapy
• Check that physician orders are appropriate and communicate with the physician at least every 30 days or when the client's status changes
• Teach, supervise and counsel the family and client on the speech therapy program and related home-care issues
• Arrange out-of-home services for procedures that cannot be done at home
• Record and report the client's response to treatment and changes in condition, and write clinical progress notes within 72 hours of each visit
• Coordinate weekly visits with the Director of Nursing and take part in case conferences and meetings when appropriate

Required profile

• Master's degree in Speech-Language Pathology; a bachelor's degree in speech and hearing therapy preferred
• Licensed or registered to practice speech-language pathology in Virginia
• At least one year of institutional experience
• Member of the American Speech-Language-Hearing Association preferred
• Two references, passes OIG, state police and national background checks, and can verify employment eligibility
• Current CPR certification, reliable transportation, valid driver's license and car insurance
