Home Health Care Coordinator (Remote)

About the role

At Home Solutions, a licensed home care agency serving the five boroughs of New York City and Nassau County, is hiring a full-time remote Home Health Care Coordinator. The coordinator organizes and oversees home care services for clients in the communities served, working with home health aides, nurses, therapists, social workers and other care professionals.

Key responsibilities

• Coordinate schedules for home health aides and other care professionals
• Ensure care plans are implemented accurately and update them as client needs change
• Maintain clear communication among clients, families and clinical staff
• Review referrals and assessments, and monitor service quality and compliance with agency policies and regulations
• Document client interactions, resolve service issues and collaborate with multidisciplinary teams
• Support caregivers with information and resources so families can navigate available services

Required profile

• High school diploma or equivalent required; an associate or bachelor's degree in health care administration, social work, nursing or a related field is beneficial
• Prior experience in home health or community-based care is preferred
• Ability to work independently, exercise sound judgment and handle sensitive information with discretion
• Multilingual skills (e.g., English plus Spanish, Russian or Chinese) are highly valued

• Organizational and time-management abilities for multiple cases and remote workflows
• Comfort with technology and remote work tools, including electronic health records, scheduling software and secure communication platforms
