Speech Language Pathologist – Home Health (Per Diem)

About the role

BAYADA Home Health Care is looking for an experienced Speech Language Pathologist to provide per‑diem home health visits for adult and geriatric patients in the Denver North Visits region.

Key responsibilities

• Conduct home visits within the assigned geographic territory.
• Perform comprehensive evaluations, diagnose speech, language, communication, or swallowing disorders, and develop therapeutic interventions.
• Create and continuously update treatment plans based on physician orders and patient progress.
• Document all observations, interventions, and evaluations using a tablet‑based electronic medical record system.
• Educate patients, families, and caregivers on rehabilitative care to promote safety and independence.

Required profile

• Current Colorado license as a Speech Language Pathologist.
• At least one year of professional experience as an SLP (home health experience not required).
• Certificate of clinical competence from ASHA or equivalent education and work experience pending certification.
• Strong computer proficiency; experience with electronic medical records preferred.
