Licensed Clinical Social Worker – Home Health Visits

About the role

BAYADA Home Health Care is seeking an experienced Licensed Social Worker or Licensed Clinical Social Worker to perform per‑diem home health visits for clients in the North Denver territory, including Broomfield, Boulder, Longmont and surrounding areas.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Provide clinical social evaluations and develop appropriate intervention plans.
• Identify and refer clients and families to community resources to meet practical and environmental needs.
• Advocate for clients and families navigating community systems.
• Participate in multidisciplinary care planning with BAYADA clinicians.
• Develop and maintain contracts with public and private agencies as resources.
• Educate patients and families on community resources to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a tablet device.

Required profile

• Master’s Degree in Social Work.
• Licensed Social Worker (LCSW or LICSW) in the State of Colorado.
• At least one year of experience working independently as a Licensed Social Worker.
