Physical Therapist – Weekend Home Health Visits

About the role

BAYADA Home Health Care is seeking licensed Physical Therapists to provide weekend home health visits in the Denver North Visits area. You will work directly in patients’ homes, helping them regain independence while enjoying flexible scheduling options.

Key responsibilities

• Perform mobility, strength, balance, and safety assessments in patients’ homes.
• Develop and implement individualized, evidence‑based treatment plans, including gait training, manual therapy, and therapeutic exercise.
• Educate clients and families on long‑term self‑care and wound‑prevention strategies.
• Document all care using BAYADA’s EMR system.

Required profile

• Current Colorado Physical Therapist license (or eligibility).
• Graduation from an accredited PT program.
• Willingness to work weekends with options for full‑time, part‑time, or per‑diem schedules.

Required skills

• Proficiency with electronic medical record (EMR) systems.
• Ability to conduct gait training and manual therapy.
• Experience delivering therapeutic exercise and balance interventions.
