Social Worker MSW – Full‑Time Position

About the role

This full‑time Social Worker position in Morgan City supports patients and families through bio‑psychosocial assessments, education, referrals, and crisis interventions. The role works within a multidisciplinary team to address emotional, social, cultural, and spiritual needs related to patients’ medical conditions.

Key responsibilities

• Conduct patient assessment interviews, provide referrals, and coordinate complex discharge plans.
• Identify and connect patients and families with community resources and supportive services.
• Maintain accurate patient records, document critical information, and communicate with the medical team.
• Participate in on‑call rotation and adapt communication style to diverse patient populations.

Required profile

• Master’s degree in Social Work (MSW) from a CSWE‑accredited program.
• Certified Social Worker (CSW) or Registered Social Worker (RSW); LMSW preferred or to be obtained within 6 months.
• Ability to perform essential duties with reasonable accommodations as needed.
• Knowledge of healthcare systems, Medicare/Medicaid, and public programs.
