About the role
The Social Work Care Manager provides comprehensive case management, care coordination, and clinical social work services to patients and families at Billings Clinic. Working within an interdisciplinary team, the role addresses medical and non‑medical barriers to promote health and well‑being.
Key responsibilities
- Review new patients for psychosocial needs, conduct assessments, and coordinate discharge planning.
- Collaborate with care managers, palliative care, and community agencies to secure resources, arrange transportation, and ensure smooth transitions across the care continuum.
- Lead patient‑family conferences, document care plans in the EMR, and maintain compliance with CMS, insurance, and HIPAA regulations.
- Participate in multidisciplinary rounds, departmental meetings, and ongoing training.
Required profile
- Current licensure as a social worker or related professional.
- Adherence to Billings Clinic policies, confidentiality standards, and ethical guidelines.
- Ability to navigate complex patient situations, including crisis intervention, substance abuse, and homelessness.
