Senior Living Occupational Therapist – Home Health

About the role

BAYADA Home Health Care is seeking an experienced Occupational Therapist to provide outpatient home‑health visits for senior and adult patients throughout Addison County, Vermont. The role offers flexible schedules with no holidays, weekends or on‑call duties.

Key responsibilities

• Perform outpatient therapy evaluations and develop individualized plans of care for patients in senior living communities or group homes.
• Collaborate with home‑health therapy partners to transition care from home health to outpatient services, ensuring timely, client‑centered treatment.
• Educate patients, families and caregivers on care plans to maximize functional independence.
• Document evaluations, treatments and discharge planning using the outpatient EMR platform.

Required profile

• Current Vermont Occupational Therapist license.
• At least one year of recent occupational therapy experience.
• Graduated from an accredited OT program (COAHEA/AOTA).
• Ability to work independently, manage time effectively and solve problems.

Required skills

• Proficiency with electronic medical records (EMR) systems.
