Outpatient Speech Language Pathologist – Home Health

About the role

BAYADA Home Health Care is seeking a full‑time Outpatient Speech Language Pathologist to provide home‑health visits for adult and geriatric patients throughout Addison, Washington, and Franklin counties in Vermont. The SLP will join a multidisciplinary team delivering rehabilitative and skilled nursing care to help clients remain safely at home.

Key responsibilities

• Conduct home visits within the designated geographic territory and perform comprehensive evaluations of speech, language, communication, and swallowing disorders.
• Develop and adjust individualized treatment plans based on evaluations, physician orders, and client goals, and document all care using a tablet‑based EMR system.
• Educate patients, families, and caregivers on therapeutic activities that promote health, safety, and independent living.

Required profile

• Current Vermont license as a Speech Language Pathologist.
• Certificate of clinical competence from ASHA or equivalent education and work experience pending certification.
• Ability to obtain and maintain required state and federal licensure for home‑health practice.

Required skills

• Proficient computer skills, including experience with electronic medical records (EMR) systems.
• Ability to use a state‑of‑the‑art touch‑pad tablet for documentation.
