Senior Case Manager RN - Home Health

About the role

Access Home Health, a part of LHC Group and part of the Optum family, is hiring a Home Health Patient Care Manager (Senior Case Manager RN) in Rogers, Arkansas. The role is responsible for the overall supervision and coordination of clinical services for home health patients, working in line with physician-prescribed plans of care and state and federal laws.

Key responsibilities

• Supervise and coordinate an interdisciplinary team to ensure continuity of high-quality care for assigned patients according to their physician's plan of care.
• Provide clinical services within the scope of practice, in line with the plan of care and with the rest of the health care team.
• Receive referrals, assign clinicians, evaluate patient orders and plot start-of-care visits.
• Coordinate patient home health benefits, medical necessity and ongoing insurance approvals.
• Ensure patient needs are continually assessed and care is individualized, reasonable and meets home health eligibility criteria.
• Review assessments and plans of care daily and consult clinicians with recommendations.

Required profile

• Current unrestricted RN licensure in the state of practice; no restrictions.
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.
• Preferred: current CPR certification or ability to obtain it within 90 days of hire, and home care experience.

• Salary ranging from $85,196 to $127,794 annually for full-time employment, depending on factors such as local labor market, education, experience and certifications.
